The Community Connections team helps to fulfill the mission of New Horizon’s Rehabilitation to empower individuals by creating meaningful opportunities in a changing environment. We do this primarily through our Direct Service Professionals (DSPs). DSPs work one-on-one with our clients who are either living at home with their family or living on their own to achieve goals that the interdisciplinary team (case managers, parents and/or guardians, and service coordinators) and the client have agreed upon. The client’s goals usually involve learning life skills such as planning and preparing a meal, managing money, making choices independently, communicating needs, interacting with others, and becoming more experienced in the community. The Community Connections objective is not to simply take clients into the community, but to help clients fully connect with and become part of the community.
